Referral & Order Review – Provider sends prescription and treatment orders.
Initial Assessment – POLAR nurse evaluates patient needs and vascular access device.
Sterile Line Placement – If needed, we place PICC or Midline catheters under ultrasound guidance.
Infusion Setup – Medications prepared, infusion pumps or supplies delivered.
Education – Patients and caregivers are trained on infusion care, safety, and troubleshooting.
Ongoing Monitoring – Nurses provide follow-up visits, line care, and progress reports to providers.
